New Patient Registration

If you live within our Practice area you may register by filling in an online form (below) and providing two forms of ID including proof of address. 

We are no longer registering patients from West Sussex - in order to maintain our high standards of care for our patients we have chosen not to register patients outside of our current boundaries.

If you are accepted you will be asked to attend a registration appointment with a doctor or nurse.

Check You Live Within The Boundary Area

If you are already a patient and are about to move to a new address please let the reception team know by completing our change of details form. They can decide whether or not you are still within our catchment area. If you are not, and you would like to stay with the practice, please let us know. All cases will be dealt with on an individual basis, by the practice manager and your named doctor.

Complete Our Patient Registration Forms

If you live within the boundary area, please complete our new patient registration:

For identification purposes, you will need to upload proof of ID and proof of address at the end of the form. Proof of ID can be a passport or driving licence. Proof of address can be a utility bill (gas, electricity or water) or bank statement – dated within the last 3 months.

Book a New Patient Health Check

Once the registration process is complete you will receive a confirmation letter from the registration department.

When you receive this, please call to book in a new patient health check with Healthcare Assistant.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.

Change of Personal Details

Please notify the practice as soon as you can of any change of address or telephone number, so that your records can be kept up to date to ensure that reminders for blood tests, health checks, smears etc. go to your correct address. It is also important that we have your correct address and telephone number should the doctor need to make a home visit.

If you move out of the practice area the doctors can no longer be responsible for your health needs. We therefore advise you to register with a surgery in the area you live as soon as possible. Please ask at reception for further details.
