practice as soon as you can of any
change of address or telephone
number, so that your records can be
kept up to date to ensure that
reminders for blood tests, health
checks, smears etc. go to your
correct address. It is also
important that we have your correct
address and telephone number should
the doctor need to make a home
visit. If
you move out of the Practice area
the doctors can no longer be
responsible for your health needs.
We therefore advise you to register
with a Surgery in the area you live
as soon as possible. Please ask at
reception for further details.
